Kindergarten Immunizations 

Ohio State Requirements

Students entering kindergarten MUST be properly immunized.  Ohio State Law requires that the students have received the following immunizations:

5 DPT
4 Polio
3 Hepatitis B
2 MMR
Varicella (Chicken Pox)*

*Starting Fall, 2006

Important Information

Immunizations can be obtained through your private physician or the Tuscarawas County Health Department. 

Students going to the Health Department must be accompanied by a parent or legal guardian

Take a copy of previous immunizations to the appointment. 

The cost is $8.00 per immunization at the health department.
